Well, either it is the ivtv-driver or it is hotplug. Have you tried to 
install hotplug from tarball?
I've tried the new firmware with ivtv-0.4.0 and it works fine, can't get it 
to work with 0.4.1 or
0.4.2, so i'm thinking of hotplug being an issue here.

I should add that I don't have much knowledge about hotplug, so I don't know 
where
to look for errors.
