I had the same problem: mythtv starts, but when starting playback, plays
static (if system is after cold boot), or tuned channel program (if tuned
with `ivtv-tune` or `ivtvctl -r xxxx`) for a second or so. The key is this
message in mythtv-backend.log:

* 2006-01-18 16:23:09.643 ChannelBase: Could not find input: Tuner 0 on
card when setting channel 3

The problem is that after updating to 0.4.2 PVR-150 card tuner layout
changes. I have similar dual TV card setup. Previously before the upgrade
MythTV setup has been using "Tuner 0" input, but now it is simply "Tuner"
- please check that in mythtv-setup and reconfigure inputs accordingly.
