Thanks for the feedback. I think I tried this. Basically I had Myth delete my cards and then I rebuilt them from scratch in the configuration area. It didn't seem to help. I was running 2.6.13-gentoo-rX for a long time. I took the oppoertunity to update the kernel to 2.6.15-gentoo to fix a USB audio problem I was having. With 2.6.15-gentoo my problem with ivtv/MthTV was gone and everything works fine. I don't know if it was some config stuff left laying around on the old kernel, or possibly ivtv-0.4.2 doesn't like 2.6.13-gentoo-rX. I'm not sure which, but I'm in better shape now.
