I'm running kernel 2.6.15.1 and todays ivtv-0.6, and it seems the problem
I have been there already when I installed everything (tested utils a
bit).

When booting up and running

  ivtv-tune --freqtable=europe-west --channel=E5

results in "snow", but when running ptune-ui and from the drop down menu
selecting "standard now PAL-BGH, #15" I will get the selected channel and
after that I can use ivtv-tune (or ivtvctl) to switch channels. But no
matter what I do without running ptune-ui once before will result in any
proper output.

The ivtv module don't seem to have PAL-BGH option, I have tried with the
parameter pal=B when loading ivtv, but that don't do any difference.

I would be happy if someone could tell how to use ivtvctl to do the same
thing as ptune-ui does when selecting PAL-BGH, so I can add that to
startup script and make that all my small scripts I use will work without
the need of running ptune-ui.
