No, you don't have a bad PVR-500.  It seems to be a common problem with 
PVR-500s
that carry a Samsung tuner, and one that is not present when using the same
hardware under Windows.

That said, I believe that at this point, Samsung tuner support in tuner.c is
most likely complete.  Most likely when they added the Samsung tuner, 
Hauppauge
changed something else in the board that requires different/additional 
steps to
fully initialize the board, and the ivtv driver is not reliably performing the
correct steps.

Things that I am positive of:
This only affects the first tuner on the card.
The first tuner on the card is the only one that has an additional FM radio
tuner
It is the fault of the driver somewhere.  Tuner 0 *sometimes* works well under
Linux but not often, Tuner 0 *always* works flawlessly under Windows when I
tested it with SnapStream BeyondTV.  (On that note, does anyone have any
reccomendations for a good Windows-based test app that is free?  
WinTV2000 from
Hauppauge only works on the PVR-250 and 350 boards.)

Things that I am rather sure of:
The only affected channels on the first tuner fall within the 65-108 MHz tuner
range of the Philips FM tuner that is also present on the first tuner.  All of
my problem channels fall in this range, but I have no signal to test with on
us-cable 95-99 which also fall within this range.
Thus, the problem is most likely connected to the fact that an additional FM
tuner is present on tuner 0.  How, I do not know.  Most likely to solve this a
complete i2c log covering *everything* on the first tuner section, not 
just the
Samsung module, would have to be taken.  With proper instructions, I 
could take
such a log in a week or so.  (The PVR-500 is currently in a Linux-only server
box, but I can move it back to my dual-boot desktop machine temporarily in a
week or so.)
