From the README file in ivtv source:
NOTE 3: The names of the inputs have changed to correspond with the names
as are used by the video4linux subsystem. This means that MythTV users have
to setup their inputs again. In particular 'Tuner 0' is now 'Tuner 1'.
 
You must use Tuner 1 not Tuner 0 in mythtv-setup.

If you can capture fine with 'cat' then I'd suspect mythtv. Are you
using the packages from debian-multimedia.org ?

Does your mythtv user have permission to read/write the ivtv device?
