It's always the obvious that I overlook!  =)

Even though it could not load the firmware, it appears that the module
was loaded.  Subsequent modprobe's were failing because the module was
already loaded.

For anyone else who stubs their "toe" on this remember to

modprobe -r ivtv
modprobe ivtv

after you have placed the firmware in it's correct location for your
OS/distro.
