[ivtv-users] Error -5 on initialization

I've had a PVR-350 for a couple of years without any issues.
 
I recently added a pcHDTV 5500 card and ever since then my ivtv is not loading. 
 
If I remove the pcHDTV 5500 card then ivtv completes the load.
 
I've tried using Gentoo kernel 2.6.16.r13 with ivtv-0.6.3 and 2.6.15-r1 with ivtv-0.4.5.  I have the same issues with both configuration.
 
Here is the ivtv output from dmesg

Has anybody gotten this to work?
