The only way I can think of is to connect the audio to the line-in of 
the PC (NOT of the PVR card) and then use the /dev/video32 raw YUV 
device with mplayer:

mplayer -demuxer rawvideo -rawvideo pal:format=hm12 /dev/video32

As far as I can tell this is instantaneous and so it should work.

If you have to use the antenna input for the audio and video, then you 
are out of luck. In that case I advice buying a cheap 'normal' (i.e. 
non-MPEG) TV-card.
