I went back to the original kernel and was able to install ivtv but I
still do not seem to have a /dev/video0 device. Is there another step I
am missing? Tried executing v4l2-ctl and it came back with device
/dev/video0 does not exist.

Axel only provides modules for the 'standard' kernels from Red Hat. There is a
package available for 2.6.18-8.1.15.el5xen but not the CentOS 'plus' kernels.

That sounds easier and I tried using Axel's ATRPMS package but I am
using the centosplus kernel for reiserfs support. When I try to install
ivtv it fails with the message that I am missing pre-req
vmlinuz-2.16.18-8.el5. I have is vmlinuz-2.6.18-8.1.15.el5.centos.plus
and I also have vmlinuz-2.6.18-8.1.15.el5xen installed but not currently
booted.
