When I had a similar problem with my PVR-150MCE, it was that the
preselector filters and switching were being set up wrong in the tuner.
The tuner.ko module had a bad/incorrect definition for my tuner. I
wound up propsing some changes to Hans that he refined into a decent
fix.
An easy option with which to experiment would be to force a different,
but very similar tuner type, on ivtv module load. This wiki page has my
particular example:

You probably have to do some trial and error, and I don't know how to
specify the type of the second tuner on a two tuner card.

I find it interesting that the second tuner found on your card has the
FM tuning capability. IIRC the ivtv driver code "knows" that only the
first tuner on a PVR-500 has FM tuning capability. Hmmm...
As for the yum problem, have yum install the kernel to resolve the
dependency: 2.6.22.9-61.fc6. You can't upgrade kernels, only install and
delete them (installonly plugin). Once you do that, you should be able
to install the kmdl package.
