I will give this a shot and report back.  Sure hope it works! =)

It was a rather sudden change.  I don't recall making any changes to the
system when it started doing this, so it certainly could be a hardware
problem.  While I had hoped to get it working again, I almost just wanted
to just establish for sure if it is a dying card so I don't regret
buying another one.
