It is my understanding that linux-headers is just a repackaging of the
header files included in the official linux kernel source. There is no
upstream maintainer except the linux kernel group and they do not
maintain a separate collection of headers. The purpose of
linux-headers is to provide a copy of the headers normally included
with the kernel source, without requiring people to install the whole
kernel source tarball. This makes sense when there are more people who
compile software against the kernel vs people who compile the kernel
itself. Theoretically, linux-headers should include every header file
located in /usr/src/linux-(version)/include and it's my opinion that
the omission by linux-headers of ivtv.h is a bug.

I have just emailed Raúl Porcel, gentoo maintainer of linux-headers,
asking if he can confirm this is the case, I will relay his reply.
