ExtraDotOutputStream doesn't properly implement dot stuffing

I've changed in head to have CRCR and LFLF before a dot not trigger
dot-stuffing.  This seems to be more compliant for POP3, but SMTP is still a bit
unclear.

Anyway, would be good to get clear references (# and section) of RFCs to be
sure.  Might need to split into multiple stuffing outputstreams if the protocols
require slightly different procedures.
