Hmm.. this is a challenge... as DISI now stands, it's undefined what doc() will
do once next/skipTo has returned false.
Even if it doesn't throw an exception, it won't return our new sentinel value.
I fear our only choice is to make a new method to return the doc, whose
semantics are defined to reflect this change? We'd provide a default impl that
returns doc() if the iteration hasn't ended, else the sentinel? Or.... some
sort of wrapper to wrap an old DISI as the new one, but then we'd need a new
DISI class to tell the difference. Hmm.
