This is probably because you are reading a document that somebody scanned, as opposed to a document that was composed with Acrobat. It is not real text, just a picture of text. Your screen reader will not read it, just like it would not read a sign in a photograph. You can try performing OCR on the document with a program like Open Book or K1000 or any OCR program that will convert the results to readable text, but your milage will vary. Sometimes that produces a readable document, but often it just produces garbage.
