I have and do use several programs to accomplish this on a regular basis.
First, Open Book will do this; Simply use the Freedom Impport Printer which
sends the document to Open Book.  I generally use the Exact View, which
preserves layout and imagery.  Then you can use the Launchables menu to
export the file to word.  
The latest version of Adobe adds export functionality as well, whenever I am
in a Word file, in the file menu, there is an option to Export file
to...offering both .docx and TXT file types.
Finally, I have used Nuance OmniPage.  It is expensive, and the interface
isn't accessible, but the functions it inserts into the context and file
menus allow you to convert any file type to almost any other file type,
again preserving imagery and layout.
