I always use alt+down arrow to open the combo box.
Then I use up/down arrow to find the option I'm looking for.
When the item I want is selected/highlighted I press alt+up arrow.
This will close the combo box and select the item.
Works great with IE8 and JAWS 12.

Press alt+down arrow to open/expand the combo box.
