I just use a headset microphone, the brand varies depending on what is on special price at any given time. I use gold wave as my recording program of choice. I record in wave format and only when I am comfortable with the final result convert it to MP3. The major thing is you need a sound card that has an option on the recording tab of what you hear or possibly wave / MP3 to obtain the desired results of hearing your own voice along with your screen reader.
Some people use a mixer but I never got that fancy since of the 9 sound cards in this house 4 of them have that capability. Each system has an internal and I also have 5 external cards.
