Ok, now I feel better.  It makes perfect sense that the command could work
differently in the 2 programs.  Some where along the way I missed that the
original question was posted by someone using Outlook Express.  Thanks all
for putting up with my confusion.

The differences isn't on how the alt+b command works on different systems
because people have been trying it on two different email clients. For
example, you are trying it with ms outlook that comes with microsoft office
and it doesn't work as a toggle with your email client where others have
been trying it with microsoft outlook express, the one that comes with
windows. Outlook express has the command as a toggle for the bcc field,
where ms outlook doesn't. so there isn't anything wrong with anybody's
system you just have to be aware of what software you are using. That's all.
