The DatabaseServerLoginModule does not require a hashed password. It supports it, but neither your configuration or the referenced tutorial use this configuration. You'll have to debug that the password being passed in is correct with no trailing spaces, incorrect case, etc. The DatabaseServerLoginModule does not log the password at any debug level so you either have to get into a debugger or use a subclass that prints out the password.
