I think it would work quite well... as long as you can also specify where the
image is (presumably you would get it via file transfer, so you'd need a
mechanism to request files or something like that.)

Exactly myself and others were working on this type of thing on the jabber.org wiki (which doesnt seem to be there anymore), but Peter kindly found the following URL to the google cache of it.

No one else really seemed interested in it up until now. If I were to produce a new spec I would simplify it much more but it gives the basic jist, i.e. you communicate a file hash utilising the industry standard cid: URI which allows the recipient to retrieve the file via standard file transfer should they want to, nice and simple and seems far more "jabber like" to me.
