I think it should be one additional *checkbox* and *field*.

*Checkbox* for enabling/disabling notes adding functionality. And *field* 
"where you could fill the pattern for notes, using variables to replace the 
values":
# If *checkbox* is unselected *field* is disabled and notes are not added in 
TestLink
# If *checkbox* is selected *field* is enabled and notes' pattern could be 
defined, but if field is empty then default simple notes are added

This will be perfect solution. But, in my opinion, at least on/off checkbox 
should be presented.

I can explain my point of view. I don't really need any notes while I'm adding 
tests' results on method level (what notes will be useful for me in this 
case?). But if it is on class or suite level, then notes can be very helpful 
and, surely, I'll be interested in some notes about tests that failed or 
succeed, and maybe execution date, time, duration, etc.
