I really don't know of anything that explains your problems.  I noticed
the device is dm-0.  I wouldn't rule out a problem with device-mapper.
What is the setup?  raid?  lvm?  The only dm-related problem I found was
fixed in 2.6.12.4: http://bugzilla.kernel.org/show_bug.cgi?id=4946 , so
that's not much help.

One thing switching to ext3 might do is to help determine if the problem
is in jfs or not.
