   Accidentally closed it, reopened.
   
   I have a comment regarding when this scenario can happen. If a client has 
already sent terminating state to true hence there shouldn't be any further 
calls expected from the same instance. Isn't that correct? Are we re-unsing the 
same instance id to create new instance which makes the recovery impossible?
