Scope of Cookie and Header Manager seems wrong

TCP Request Defaults do work, after all - my mistake.

Inspection of the code shows that the problem with the Cookie Manager etc seems
to be that they don't just store variables in the test element, the Managers are
invoked at run-time. 

Defaults are cumulative, which seems correct, but Cookies etc should be local
only (otherwise impossible to cancel inherited cookies).

The child cookie manager should override any at the sibling level.

But I could not work out how to fix this.
