Version : jmeter 2.1.1
No message on the console window.
Saving the results to a log file is not doable. It is one request out of thousands. When I send less requests, there is no missing request. What is CSV? If it is one of the data writers, I did not use one. I used the command line to choose a file.
What should I be looking for in the log file?

Which version of JMeter/Java?

Any messages in the console window?

Have you tried saving the results to a log file?
CSV is cheapest on disk space.

Or you could add a summariser - this just writes to the log file every
so often, and at the end.
