Sorry for the long delay to reply, I was on holidays when you sent your
mail and much work to do since coming back.
I've had a quick look at MW options (
http://www.mediawiki.org/wiki/Manual:Configuration_settings#Users ), and I
didn't find an option to require authorization to create an account.
If you know how to do this, I'll use it.

I don't think it will too much a problem for the system (MW is clearly
designed to handle huge systems, 17M users registered on enwiki)
 I would need to upgrade the wiki to a newer MW (1.19.1 probably), but need
free time to do so...
Hopefully, the new MW will help
