I just tried to implement this.

Unfortunately, Apple has (yet another) bug in their mouse event handling.
I have submitted a bug report to Apple.

But in general, most applications are not trying to implement six (ten?)
degrees of freedom with a single button mouse :-)

Apple *tries* to make CTL_LEFT be a popupTrigger ... Sun also made some
feeble attempts ... both failed miserably.

The only way to get this to work is for individual applications to
implement this themselves.

ALT_LEFT drag is mapped to MIDDLE_DRAG (per Sun's human interface
guidelines for dealing with single button mice). And MIDDLE_DRAG is mapped
to zoom functionality.

So, this is not available.
