And actually, I just tried it, and ESC doesn't work as a modifier (on Windows at any rate, I haven't got a Linux box with a UI to try it on): if you don't release the ESC before you press the mouse button, you don't get an extension from the node you create (presumably ESC up is deselecting as well as ESC down, but that's a guess). So while you could, in principle, keep ESC pressed or use it like a modifier to add individual POIs, you can't use it like this to start a new way from another point (either in open space or from another node or way). Another strange thing is happening, and this may be a bug. You said if I double click or click again on a node from which an extension is currently pending, it stops extending. Well, that does indeed seem to happen, and I can start a new way in open space. But if I try to start a way from an existing node after doing that, it won't do it. I have to click twice on the target node before it will start extending from it.
