Guess this is my day to plug Jason Hunter's book.   Jason's text, Java Servlet Programming, O'Reilly, provides a good tutorial on multipart requests -- i.e.: file uploads. Visit Jason's site at www.servlets.com (?).   If you don't have the text, I'd recommend you get it.
