Hi Jean,
You should be using sessions for this. You could try the follwing steps:

1) Make the first html page which contains ur form as a jsp. In this jsp get access
to the 'session'. 'Put' the values the user inputs into the session object.
2) In the second jsp, after the user has agreed to the license agreement, you can
'get' the values you have put in the session previously and process it further for
db insertion etc.
