I think its better to use two different forms.
That way your JSP code will be simpler.
You can reuse the HTML though if its same or similar for both forms.
Also, if some JSP / Java code is common between the two forms, you can put
that into helper class(es) and call the methods from both forms as needed.
So you get reuse as well as maintainability :)
