This really depends on your design and performance goals. Will you pages be
mostly static or dynamic?
If your pages are mostly static then you can easily use an include directive
or action. Knowing which one to use is well explained in:

If you would like to use the same form you should look into custom tags. JSP
pages can handle application logic through the use of JavaBeans components
and custom tags. If JavaBeans components and custom tags are designed in a
general manner, they should be reusable in other portions of the application
