I am grateful to everyone who has responded to my query regarding the
aforementioned dissertation. I would especially like to thank John for his
clarification in terms of its (non) publication, as well as his advice on
ordering
via a university library. As far as the former point is concerned, I was
evidently misled somewhat by the different year which appeared in a particular
footnote citing this work.
