There's as many combinations out there as there are programmers who want to improve upon the web server's implementation of session tracking. I've seen the same application use up to three of URL rewriting, cookies, hidden form fields, and path segments. Sometimes the developer left the application server's session tracking turned and decided to ignore it and write their own, or sometimes an application will be a combination of a couple of different third party applications, each with its own session tracking scheme.

You'd be surprised how many programmers decide to invent their own proprietary session tracking schemes.
