No icon for firefox on bottom bar

SUMMARY
I created a plasma-applications.menu under ~/.config/menus/ and removed the
applications-kmenuedit.menu.

STEPS TO REPRODUCE
1.  Remove ~/.config/menus/applications-kmenuedit.menu
2.  Create custom ~/.config/menus/palsma-applications.menu
3.  Reboot
4. Login and you should get same result as screen

OBSERVED RESULT
Applications which were running before reboot are running and accessible with
ALT+TAB, but as show on screen, there is no icon.
There is also the bottom bar which is not attach on bottom and full witdh
screen.

ADDITIONAL INFORMATION
To get back working view, under session
- delete the plasma-applications.menu file
- logout then login
- recreate the plasma-applications.menu
