Due to a hardware issue and/or Koha performance issue, we have a number of
duplicate transactions in the system. I suggest that Koha not allow a
circulation transaction to be recorded in issues, action_log, or statistics if
another transaction exists that exactly matches the timestamp, borrowernumber,
and itemnumber.
