I wondered about that behavior for record-level holds myself - it's present in main, and was actually one of my motivating factors in writing this patchset. I didn't think about the use case where a library might want to prevent checkout if there is a pending item-level hold but not a pending record-level hold, but I agree that would be a sensible behavior, and is worth adding a follow-up patch for - stay tuned! The testing error was a copy/paste error - the SCO was checking the SIP preference instead of the SCO preference.
