You were right on both counts. - Replacing a record's covers no longer removes its item level covers. - With AllowMultipleCovers enabled, existing covers are removed once per record, so all the images in the zip are kept. With it disabled, covers are always replaced, even without ?replace=true. Bug 42405 had the same item cover problem in POST /biblios/{biblio_id}/cover_images, fixed there too. And yes: the staff upload tool uses the API in bug 42804. The follow-ups also tighten the path checks, finish the job exactly once, and add tests.
