I had very good success writing a letter to the president of Pac Bell a few
years ago.  This was a different problem (I wanted them to run a new cable
on my street to replace an old noisy one with no more extra pairs, so I
could get DSL).  I called 611 repair MANY times with no results, but one
time the person told me to write to the president of Pac Bell and gave me
his name and address.  I was skeptical but he assured me it would get
results.  I wrote the letter and one day the following week got calls from
the chief engineer in San Francisco, the regional engineering director in
San Diego, and a local installer.  The next day there were three trucks on
my street replacing 1/2 mile of cable. I was quite impressed.

You might give it a try.
