I found (just) one! All those others I was thinking about were probably
ISA <heh>.

The one I found came out of a compaq windows box -- don't know whether
it's a winmodem or what?

chip is labeled
  conexant rh56d-pci

pcb has printing that includes "F-1156"
backside label says model F-1156IV/R9
 + FCC Reg No DK4TAI-27391-M5-E
