Check your dmesg output to make sure that the device is actually getting assigned.

Also, make sure that your SCSI cable has a terminator. Since the tape is the only thing on the cable and these were both inside another machine, you absolutely need a terminator.

The cable should go:
    |      |    |     |    |
SCSI|------|TAPE|-----|Term|
CARD|      |    |     |    |
    |      |    |     |    |

The SCSI card should be at one end of the cable, the terminator should be in the *last connector* at the other end of the cable, and the tape should be somewhere in between.

If you accidentally reversed the cable or there was another device which did the termination before, it can cause mysterious initialization problems.
