I would argue that it is impossible ever. The combination of driver's license and credit card is as close to universal PKI as you are going to get and even *that* doesn't work.
Not all the problems are technical.
I remain optimistic about 'niche' PKI rollouts within limited domains. For example, the DoD with their smart card systems *is* able to force everyone to use the same CA.
