Hmm, don't know where my previous binary went. Trying to rebuild the rpm
or tgz is giving some problems.

/usr/sbin/sbminst that can write out the needed floppy.
Running
  /usr/sbin/sbminst -d /dev/fd0
just worked for me.

You may even be able to get by by downloading the prebuilt sbminst,
doing a chmod +x and running that!
