It is probably one of the settings within the mailbox. When you recreate the
mailbox it resets it to the default.
What exactly does the caller get when they call that extension?

i.e. is call first answered by the custom menu and the caller enters the
extension number? When the phone ext does not ring, what does the caller
hear? For instance does the system say "that extension is not available" or
something like that?

The mailbox features I would check are 4, 1- "call transfer options",
3-"message reception mode", and 4-"incomplete handling status".

Sometimes the users turn things on/off by mistake.

without deleting the mail-box because people have to record their names and
greeting messages all over again.
