Thanks everyone for your help on trying to get the CID working on my system.
I figured it out, and it was a STUPID mistake on my part.

I found a friend who had an 816 about 25 miles from here, so drove out to
test the card there. It worked fine, which had me confused for a little
while, then it struck me how I had wired the incoming line at home. I had it
first go to my home automation computer so that it could announce CID names,
then from the top jack of the computer's modem, I went to the KSU. It turns
out that the modem did not have a straight pass-through from one jack to the
other, it only passed the ring signal. I just inserted a splitter in the
bottom jack, for the time being, and now everything is happy.

Sorry to chew up bandwidth with such a dumb mistake, but it just did not
occur to me to check what devices might be upstream that could chew the CID.
Duhhhhhhhh....
