I did the programming at the phone. then, I hit the fwd button to dnd and to
fwd to try each. in every case, the phone would still ring when called from
a co or from another extension. that's why I believe i am not using cos or
something else (system setting) correctly.

The forwarding is a sort of 2-step process. First you program the button
for the type of forwarding you want it to do, then you can toggle it on/off
by pressing the button while the phone is still on hook.
If you have programmed both DND and forwarding, then the button will toggle
between Off/DND/Fwd.
Here are some examples:
To program the button to forward calls busy/no answer to voicemail,
go off hook (pick up handset)
press FWD 5 165
hang up
To program the button to forward calls to an outside number
go off hook
press FWD 6 9 [the number to dial] <you need to put the 9 in to grab an
outside line>
hang up
To program DND
go off hook
press FWD 1
hang up
To cancel all FWD or DND
go off hook
press FWD 0
hang up
Now, to turn it on
Do NOT pick up the phone
press FWD button
The display will tell you what it is set for.
Press FWD again to toggle through the DND FWD or OFF.
The default for the LED is a solid light is DND and flashing light light is
FWD (that can ge changed in the -4 systems though)
