All airlines that accept dogs have to put them in acclimatized holds -
continental is just smart enough to advertise that.  The USDA has rules
about handling live critters.  Most airlines are at about $175 for a size
100 or small kennel with up to 25 lbs dog and combined.  The next size up
jumps to about $200 with a big jump to $300 for the medium or size 300
kennel.  We ship every month year round as well as ship internationally a
few times a year.  Weather does play a part as does breed of pup.  From
where to where is this pup to go - I know most of the airlines schedules and
can let you know of the shorter flights.

No far....call the airlines, should be less than 200 due to size.  Weather
and fagility of the puppy may play a factor though.  Contintntal has climate
controlled cabins....
