I have heard it used to fit both of those definitions. I have also heard "Went South" as meaning the same thing as "Belly Up", "Ten Toes Up" or "Six Feet Under". IE... Dead as a doornail. When a business "Goes out of Business" it is sort of "dead as a doornail" too so it is sort of related to that side of the definition. However, when I think about it, when your boobs go south they are really on their last legs too...so to speak. Hee hee.
